Note: Names and details are provided for your information only and are provided solely by the physicians. Decisions regarding choice of physician and treatment are a patient’s responsibility, as is all communication and interaction with listed medical professionals. Physician’s listed on the Physician Locator pay no fee for inclusion. Some physicians on this list may purchase products from, provide consulting services to and/or be a party to a co-marketing agreement with Relievant. Relievant makes no representations or warranties regarding, and shall not be responsible for, the competencies or skill level of any of the physicians listed on the Physician Locator or the quality of their procedural outcomes.
On April 1, 2023, Relievant will update the criteria to be listed on the Physician Finder:
- These new criteria will require physicians to have 1) successfully completed their initial training and 2) performed at least 5 Intracept Procedures within the preceding 6 months in order to be listed
- To be added back to the Physician Finder, a physician must perform at least 5 Intracept Procedures within the preceding 6 months
Additionally, Relievant Physician Faculty that train and educate their peers at Relievant-sponsored programs meet eligibility requirements.
Relievant reserves the right to remove and/or exclude any physician from the Physician Locator, immediately and without notice in its sole discretion in the event Relievant becomes aware of (i) any alleged or suspected fraud or abuse or violation of law, arising out of any activity involving the physician, (ii) any alleged or suspected violations of any federal or state health care program requirements by the physician, (iii) any threatened, proposed, or actual revocation of license/certification, suspension, exclusion, or debarment of the physician, (iv) any subpoena, legal process, civil investigative demand, discovery request, or similar request for information involving the physician, and/or (v) any facts, allegations or situations of a similar nature.
